System QA Issues track manual QA findings, sprint carryovers, route problems, expected results, actual results, and verification status.

Workflow

1. Create a QA issue with source, severity, module, optional route, reproduction steps, expected result, and actual result.
2. Move the issue through TRIAGED, IN_PROGRESS, RESOLVED, VERIFIED, CLOSED, or WONT_FIX.
3. Add resolution notes before verification or closure.

Guardrails

The tracker is local issue management only. It does not add production deployment, external monitoring, AI triage, or customer-facing reporting.
